Monticone reportedly began stealing fentanyl in June 2020 for his own use. To make it appear all the narcotics were there, the former nurse gained access to secure storage areas, stole vials of fentanyl, used a syringe to remove the drugs from the vials, and then reinjected saline into the vials. Investigations also revealed that from June to October, more than 75% of the fentanyl administered to patients at the Yale REI Clinic was contaminated with saline. Additionally, some of the vials contained diluted fentanyl, while others simply contained saline and no medication. As previously stated, Donna Monticone was embroiled in scandal in 2020 when she switched patients from the pain reliever fentanyl prescribed to saline.
